When I search by Artist, MC displays a page with the artist's photo and bio from last.fm at the top. Problem is that I cannot find a way to add an artist photo if one doesn't exist in last.fm. I can do an "edit" to add the artist bio, but there's no similar feature for the photo that I can find?

A related issue: For one artist that did not have either a photo or bio in last.fm, I actually went ahead and added one to the last.fm site. That photo and bio now appear if I search for that artist on last.fm, but they still don't show up in MC despite doing a complete "level 3" rescan. Why not?

MC doesn't offer a sperate photo update feat on it's self; It uses the last.fm photo library, if you don't like an artist photo you could (like you have done already) upload a new one or vote for one of your liking on last.fm. In the next artist update cycle MC will pick the photo that is at that moment on top.
MC syncs 3.3% of your artists per scan with last.fm and logs the last date it has done this task. This means a recent artsist update will require 30 scans to re-update its details from last.fm.
