Accord. to #4 in OP, it may be a firewall on the server machine blocking 4040, so lets try to get your lan machines seeing the server first. In a browser, open your server lan IP with the 4040 port (eg. 192.168.1.213:4040), and see if you can now access the server over lan. ( I just realised that I haven't installed Subsonic in so long, I cannot recall if the install prompts to allow exceptions to firewall during installation!). Once lan machines can see the server, we can proceed with testing the wan connection and adjust the router, which may takes a few reboots to 'take' .
edit: Accord. to Subsonic Getting Started Doc:
"Firewall. You might be running firewall software (on the Subsonic server) which blocks incoming traffic. If this is the case, try first to turn the firewall off. If this helps, turn the firewall back on and configure it to allow incoming traffic to Subsonic. Note: The Windows installer version of Subsonic will automatically configure the built-in Windows Firewall."
So Windows firewall should be good, but if you have another firewall running, it will need configuration.
