JW FLV player does not load in IE (6 or 7)

I have JW FLV Player 3.16 installed, but it will not load in IE
has anyone seen this issue?

the box is a flash object, but it will not load
it is a grey box with a spinning wheel animation.

works in Firefox 3 and Safari without a problem
